Dad has a daily pill sheet, which has a place for him to "check off" his pills when he has taken them.  This was originally intended to make sure that he was taking his pills, now it serves as a short medical history of each day.

On the sheet, we now record his pills, his weight, his blood pressure, his exercise, whether or not he did a #2, and any other pertinent medical information such as "dizzy" or "tired."

Since I have this information recorded every day, I can look back if a doctor asks for any specific symptoms or issues over a period of time.

Yesterday, he was dizzy and weak in the early part of the day, but he recovered well and felt good last night.

Around midnight, Dad went to see if he had taken his pills and saw the notes.  He immediately woke me up and asked me why he had been dizzy and weak.  Was he having a heart attack? Was he dying?  He worked himself into a frenzy worrying about his health.

So I got up, took out a blank pill sheet and wrote, "Excellent day.  Dad felt wonderful!".  He read that, smiled and went off to bed.

Now I will have the "reality" sheet for me and a "good news" sheet made just for Dad to see!
